It's important people living in Rhondda Cynon Taf, Merthyr Tydfil and Bridgend have access to the services and support that they need.

Transforming services to support people to live happier and healthier lives

People deserve to live long happy and healthy lives. To ensure this happens, we need to find ways to secure better health outcomes for people of all ages and backgrounds.

By testing new services, and working closely with citizens and health and social care staff, we can start to see what improvements will be beneficial for people in the long-term.

 

 

Transformation Fund

In 2018 the Welsh Government established a £100m Transformation Fund to help Regional Partnership Boards support its ‘A Healthier Wales’ ambition. 

The purpose of the Transformation Fund is to improve health and social care services by scaling up models that are successful, and replacing less successful or outdated ones.

Through this fund, we were able to establish new health and social care services based on what people told us they needed.

Then in 2019, we secured  an extra £22.7 million to further accelerate and expand the transformational projects taking place across Rhondda Cynon Taf, Merthyr Tydfil and Bridgend.
